Law Firm Usage Guide- Scenario C

Scenario C: Client completes/submits Client Enrollment Form at final signing, law firm registers clients via Fax

This method cannot be used to submit any non-healthcare related information to LegalVault. If you would like to add legal/financial documents, you would have to access the client’s LegalVault account after it has been created and submit them online.

Starting October 1, 2010 using this method will incur an additional $4.00 data entry fee per enrollment.


  • Step 1 – Have client complete the Client Enrollment Form
    You may obtain copies of the Client Enrollment Form by signing in to your Site Manager account by visiting legalvault.com and clicking on Partner Login on the top right of the screen. (If you forgot your firm’s login information, please contact us). Once you are signed in, you may access the form under the LegalVault Client Manager, Download Forms.

  • Step 2 – Complete the Client Registration via Fax Cover Form
    • You may obtain copies of the Client Registration via Fax Cover Form by signing in to your Site Manager account by visiting legalvault.com and clicking on Partner Login on the top right of the screen. (If you forgot your firm’s login information, please contact us). Once you are signed in, you may access the form under the LegalVault Client Manager, Download Forms.
    • Complete the form as indicated and fax along with copies of client’s advance directives. LegalVault will add all of the data from the enrollment form along with the client’s advance directives to the new account
LegalVault typically prepares and mails wallet cards within 14 business days of the request.
