Scenario A: Client completes Client Enrollment Form at least 10 days prior to final signing
We recommend providing the Client Enrollment Form to your clients early in the estate
planning process. The benefit to following this routine is that you are able to provide the
wallet cards to clients at final signing.
Step 1 – Have client complete the Client Enrollment Form
You may obtain copies of the Client Enrollment Form by signing in to your Site
Manager account by visiting
legalvault.com and clicking on Partner Login on the top
right of the screen. (If you forgot your firm’s login information, please
contact us). However, if you are an Zola Creative member, you can certainly sign in from the Zola Creative website.
Once you are signed in, you may access the form under the LegalVault
Client Manager,
Download Forms.
Step 2 – Sign in and register client
Log in to your Site Manager account by visiting
legalvault.com and clicking on Partner Login on the top right of the screen. (If you forgot your firm’s login information, please
contact us)
Under LegalVault Client Manager option to the left of your screen, click on Enroll New Client
Complete requested information including Name, Date of Birth, Username and
duration of the Subscription Term and click on Enroll Client.
Step 3 – Store client Information
Input other information, such as Emergency Contacts, Primary Care Provider, Allergies and
Medications, from the Client Enrollment Form under each respective tab under Documents and Data > Medical.
Step 4 – Submit Client Enrollment Form to LegalVault
Scan the completed and signed Client Enrollment Form and submit it using the “Upload
signed Client Enrollment Form to activate client account” icon on the
Administration page.
At this point, you may request the client’s wallet card by clicking on the icon Request
Plastic Wallet Card from LegalVault. LegalVault typically prepares and mails wallet cards within 14 business days of the
request.
Step 5 – After final signing, submit copies of documents to LegalVault
Upload healthcare related documents such as healthcare powers of attorney and organ
donation forms, go to the client’s Documents and Data page > Medical tab > and click on Advance Directives. Use the form at the bottom of the page to load the
documents. .
Upload legal and financial documents, go to the client’s Documents and Data page > Legal/Financial tab > and click on Legal/Financial Documents. Use the form
at the bottom of the page to load the documents.
